  • Sponsor Name including link to website (if provided) will appear on our website 
  • Sponsor Name/Logo will be placed on the banner that will travel with the team and be hung outside the dugouts

  • Have check for $100 made out to “Middletown Monarchs”
  • Hand in form when balances are due (early April)
